Students will need the following information before submitting their community service hours:
- Student ID #
- Date of Service
- Hours Completed
- Organization Name
- Description of Work
- Contact Person
- Contact Person's Email
- Contact Person's Phone Number
It is recommended that students submit your hours as they are completed.
You must be logged into your Google Email to submit the form. You will be emailed a copy of your submission. It is recommended that you save your copy of the submission in a folder in your email. Watch a quick tutorial on how to submit your hours.
